2024-2025 Maine West Middle School Nordic Team Registration Form

Open to students from all area schools, grades 5-8, who are ready to commit to a more structured team atmosphere focused on building fitness, improving skills, and having fun. Racing is encouraged but not required. Program will run 3 days a week from December through February on Tuesdays, Thursdays, & Fridays (excluding February vacation) from 2:30pm-4:30pm. Wednesday Night Lights will provide an optional 4th day of practice. Race options include midweek and weekend Maine Junior Ski League races, Non-Stop Nordic races, Bill Koch League Fun Races, and the Bill Koch Festival (3/1-3/2) for those who are interested.

Program fee is $150. Equipment provided by the Bethel Outing Club. Trail pass provided by Inland Woods and Trails for youth living in SAD 44, SAD 17, and RSU 10 areas. Payment plans and scholarships are available.

2024-2025 MWPP, Bethel Outing Club, MICA Project Fund, Inland Woods and Trails, and Gould Academy Nordic Ski Waiver

In consideration for the rights and privileges associated with participation in the MWPP Nordic Ski Team, I, as parent of a player, acknowledge and agree to be bound by the following:

1. Identification of Risks. I, as parent of a player, understand that participation in outdoor physical activities, including but not limited to, preparation for, participation in, and coaching of activities, competitions and clinics, involve risk of serious injury, including permanent disability, death and other losses, due to inactions or negligence of myself or others.

2. Assumption of the Risk. I, as parent of a player, agree that I am responsible for my child’s safety while participating in activities associated with MWPP and that such responsibility includes participation only; a) when my child is both physically and psychologically prepared to participate safely, b) after fully familiarizing my child with the venue before beginning the activity, and c) while using the equipment of a type and condition reasonably necessary to safely participate. I, as parent of a player, assume all risk connected with and responsibility for any injury or loss connected with my participation.

3. Waiver. Aware of the risks and willing to assume them, I, as parent of a player, hereby waive, release, and agree to hold harmless the MWPP officers, directors, employees, agents, coaches, trainers, doctors, officials, event organizers, or sponsors (Released Parties) from any and all claims by me for any liability, injury, loss, or damage in any way connected with my participation in activities associated with MWPP except where caused by the gross negligence or willful or wanton misconduct of any of the Released Parties. I, as parent of a player, intend for this waiver and release to also apply to any relatives, personal representatives, heirs, beneficiaries, next of kin, or assigns who may pursue any legal action or claim on my behalf.

4. Insurance: I, as parent of a player, currently have, and agree to maintain throughout the time that my child trains and competes, valid and sufficient medical and accident insurance. I, as parent of a player, understand that this is my sole responsibility and release all persons and entities from providing this coverage for me.

I, as parent of a player, have read this release and waiver of liability agreement and understand that I am signing this freely and without any inducement or assurance of any nature, and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.
